Evaluation of Managing Heart Failure @home
Heart failure is a major cause of premature death. It can seriously impact an individual’s quality of life and result in frequent use of healthcare services. Rates of heart failure are increasing due to an ageing population and the prevalence of long-term conditions.
Managing heart failure @home is an innovative approach that aims to support people to better manage their heart failure condition and keep well at home. It uses personalised care, remote support and monitoring and increased integration of primary and secondary clinical services to allow a person to monitor and manage their heart failure symptoms. This approach has been tested across ten sites with support from NHS England.
Commissioned by Health Innovation South West, MTech Access has evaluated the effectiveness of the Managing heart failure @home approach. The evaluation report was published in December 2024. Its key findings include:
- Patients experienced an improvement in health-related quality of life, heart failure related symptoms and ability to manage their heart failure.
- Patients accessing MHF @home experienced a reduction in their use of primary care services.
Whilst many test sites saw a reduction in patients’ use of secondary care services, additional analysis is being undertaken to better understand the variation between sites.
