Adult Social Care Network – Managing Deterioration

We hope you will join us on Thursday 28th November for our next FREE online event offering peer-to-peer support and showcasing, celebrating, and sharing ideas across urgent and emergency care and adult social care.   
 
 
Are you interested in exploring how the Adult Social Care sectors across Devon, Cornwall and Somerset can collaborate to improve care services and outcomes?  
 
Would you like to discuss ideas in a safe and open environment, share your knowledge, and find out what we’re each doing to manage deterioration in our region?  
 
This event is designed to explore the ways in which Adult Social Care can support the NHS to prevent/reduce avoidable admissions, improve the transfer of care, improve the patient flow, support with reducing the length of stay in hospital and look at how research, innovation and tests of change can support the long-term improvements and impact of patient experience with the support of the Adult Social Care sector.  
Our November event will cover: 
  • Martha’s Rule – what is it? How is it locally relevant? What are we doing to test and implement the approach?
  • The PIER (Prevent, Identification, Escalation, Response Framework) – what is it? How is it being explored across the south west to deliver system wide improvements?
  • An opportunity to feedback on the strengths, opportunities and resources when it comes to keeping the frail population at home.
  • Aligning, sharing and learning in a safe and open environment from across the region
  • Health Innovation Network: Who we are and how we can support you

Who should attend? 

This is an open, inclusive and informal opportunity for people across the following sectors to connect and learn from each other: 

Care Providers, Registered Managers, Local Authorities, NHS staff, ICSs, Care Co-ordinators, OTs, Charities, Care Homes, Domiciliary Care Providers, Project/Programmes Managers, Improvement & Transformation teams, Primary Care, Trusts, Physiotherapists, Commissioners, Care workers, Carers, friends and family members of people being cared for and/or experts by experience.  

Why should I attend and what will I get out of this event? 

You will learn about what other people in the South West are doing to support the prevention of falls across Health and Care services and care provision. 

You will be able to meet new connections, network and share ideas and good practice.

Our Network events are all FREE, friendly and conversational. We welcome a wide range of people and you can engage as much as you like through the chat function – there is no pressure to speak if you don’t want to.
