About Digitalisation of Social Care
The Digitalisation of Social Care (DiSC) project was managed by Health Innovation South West between April and September 2023, and has since been handed back to NHS Devon Integrated Care Board (ICB), who will continue to see through the project to support adult social care providers in Devon to adopt Digital Social Care Records (DSCRs).
Since beginning the DiSC project with NHS Devon ICB in April 2023, we have successfully onboarded 114 adult social care providers with a digital supplier and digitised 88 providers across 730 CQC-registered organisations in Devon. We would like to thank all those involved for their efforts and commitment to the adoption of digital systems in adult social care.
NHS Devon ICB will be able to support care providers with accessing funding to access digital solutions that will benefit those in receipt of care home services, their families, and the staff who care for them. For any queries relating to DiSC please contact Sian Bunce, Digital Transformation Programme Manager, NHS Devon ICB.
Join our Adult Social Care Network

Health Innovation South West remains committed to supporting the care sector to adopt digital systems to improve patient outcomes and increase information-sharing between health and social care. We have created a new Adult Social Care Network, covering Devon, Somerset, Cornwall and the Isles of Scilly. The network, which builds upon the popular South West Care Home Network which closed 2023, is growing our audience to invite and include domiciliary care and a wider audience of adult social care staff, providers and charities. We are hosting a series of free online sessions offering peer-to-peer support and showcasing, celebrating and sharing ideas across adult social care, including digital social care records and the use of technology.
